To become appointed, you can complete our online Producer Questionnaire. Be sure to have your entity/individual producer license(s), E&O Declarations page, and agency W-9 available. Once you complete the form and attach the necessary documents, you will be able to provide an e-signature, click “Continue,” and the form will be sent to us automatically.
You can also download the Producer Questionnaire to fill out. Once the form is complete, you can email it with a copy of your entity/individual producer license(s), E&O Declarations page, and agency W-9 to brokerageops@asperains.com.
We will send you a Producer Agreement once proper documentation is received and reviewed.
Your Agency’s Owner or Officer will need to review and sign the Producer Agreement, then return it to brokerageops@asperains.com.
Once we receive the signed Producer Agreement, we will be able to bind coverage for you!
